Welcome to the City of Houston's Birth and Death Certificate online ordering system. Requests will be processed between Monday through Thursday, 8:00 am to 4:00 pm, and Fridays, 8:00 am to 2:00 pm.
Please note the following:
- 'Full size' Birth Certificates are available only for births that occur within Houston city limits and some limited surrounding areas.
- Long Form Birth Certificates are NOT available for the following hospitals: St. Luke's Health - The Vintage Hospital, HCA Southeast, Houston Methodist Willowbrook Hospital, Memorial Hermann Cypress Hospital, Memorial Hermann-Sugarland, Memorial Hermann - Pasadena
- If the birth occurred outside of the City's registration district but within the State of Texas, a Birth Fact will be issued instead; this is a certified legal document typically acceptable for the same purposes as a Birth Certificate, except in the case of Passport applications.
- Death Records are available only for those deaths that occur within Houston city limits and some limited surrounding areas.
- Valid ID MUST be attached to your application at the time of the request and you must be a qualified applicant (immediate family). If ID is not attached at the time of the request or does not meet the requirements of the State of Texas, or you are not a qualified applicant, your order will be denied. SAME-DAY PROCESSING Expedited service, available for pick-up without waiting in person. Same-Day Processing is available if the complete order is placed by 2:00 pm (CST) Monday-Thursday or 12:00 pm (CST) Friday; these orders are typically ready for pick up 2 hours after a complete application is submitted. Any order placed after those times will be available for pick up after 10:00 am (CST) the next business day. You will receive multiple emails: the first email confirms the order was received; the final email indicates the order is ready for pick up. Your order will not be ready for pick up until the final email is received. Houston Health Department Pick Up Hours: Monday-Thursday 8:00am-4:00pm, Friday 8:00am-2:00pm Note: Orders not picked up within 30 days of the order being completed will be mailed once the address is verified. If there is no response after a few attempts (email and /or call) the certificate will be mailed to address listed in application. NO REFUNDS or REPRINTS will be issued if the certificate is lost in transit. REFUNDS will not be allowed after the order has been processed.
